Preparing for Your First Session
Adults

Complete all your intake paperwork in the Theranest link sent when you made your appointment. If you have not done this- please arrive 20 minutes early to complete the paperwork at the office. You cannot be seen until it is completed.

Complete your own call to your insurance company to verify your benefits and compare to our information.

Bring your Payment Form, Drivers License or ID card, Insurance Card and list of any Medications your are taking along with your prescribers name.

These documents will be copied when you check in and your payment will be collected. We accept Credit Cards which include a 4% fee. You can always choose to pay in cash or check to avoid this fee. HSA and FSA are accepted.

Do not bring children younger than 10 years of age unless you bring another adults to supervise them in the waiting room. We do not provide childcare services and it against our policy to leave children younger than 10 unaccompanied in the waiting area.

Your therapists will make you comfortable and begin the "assessment" by gathering the reason for your visit, your biological, social, family, medical and mental health history along with your symptoms and areas you would like to improve.

A preliminary diagnosis and goals of treatment will be discussed including recommendations for types and module of treatment that best fit your needs.

Your next session will then be scheduled.

The following session will include a formal treatment plan for your care, a discussion of modalities available and be followed by sessions as mutually agreed upon by you and your therapist.
Children

If your child is too young to wait by them self (younger than 10), please bring another adult to the initial session to watch them while you complete the first part of the assessment (approx. 25 minutes) alone with the therapist. We do not provide childcare services and it against our policy to leave children younger than 10 unaccompanied in the waiting area. The second part of the assessment will be with your child.

Your child's therapists will begin the "assessment" by gathering the reason for your visit, your child's biological, social, family, medical and mental health history along with their symptoms and areas you would like to improve (around 20 minutes).

Your child will meet individually with the therapist for the 2nd part of the assessment (around 20 minutes) where any age appropriate information will be collected or observed.

A preliminary diagnosis and goals of treatment will be discussed including recommendations for types and module of treatment that best fit your needs (around15 minutes).

Your next session will then be scheduled.

The following sessions will include a continuation of the assessment if needed, ruling out any other diagnosis' and developing a formal treatment plan for your child's care. Sessions will continue as mutually agreed upon by you and your child's therapist.
Couples

Assessments for Family Therapy takes 2 sessions. Only Adults are requested to be at the initial session. One person must be the the identified client for insurance billing. Do not bring children younger than 10 years of age unless you bring another adults to supervise them in the waiting room.
